Health For Life is a program that aims to help people diagnosed with a chronic
disease with self awareness and management of their health.
Clients of the program will have access to information, support and resources to not only manage the chronic disease, but improve and maintain their health and wellbeing.
The program will involve:
- Comprehensive health and lifestyle interviews
- Client setting goals for what they want to achieve from the program
- Clinical consultations with relevant BCH Allied Health Service Providers i.e. Physiotherapist, Podiatrist, Counsellor, Dietitian.
- Ongoing consultation with the Clients own GP
- Access to health coaching and local support groups/services.
Location of Service
The Health For Life office is based at the Banyule Community Health (BCH) West Heidelberg site. The Health For Life program incorporates BCH services and programs offered at the West Heidelberg and Greensborough sites. Any services or programs offered through the Health For Life program, but external to BCH are held within the Banyule local government area.
Hours of Operation
The Health For Life office operates from Monday to Friday, 9am - 5pm. Services offered through the Health For Life program vary in operation hours; please consult the Health For Life Service Coordinator 9450 2051 or banyule@bchs.org.au for more information.
Eligibility Criteria
The Health For Life program is open to anyone residing in the City of Banyule who is the in early stages of, or recently been diagnosed with at least one of the following chronic diseases:
- Cardiovascular disease (i.e. Ischemic Heart Disease)
- Respiratory disease (i.e. Asthma or Emphysema)
- Osteoporosis
- Diabetes Type II
They must also have at least two of the following lifestyle risk factors:
- Poor diet/nutrition
- Smoker
- Excessive alcohol consumption
- Inactive
- Socially isolated
- Overweight
- Inadequate self management skills
- Depression/Anxiety
- Pre-Diabetes
- High Cholesterol
- High Blood Pressure
Costs to Clients
There is no cost involved in joining the Health For Life program. However consultation with Allied Health Service Providers will incur standard BCH fees. Fees are minimal for healthcare/pension cardholders and low income earners. If Health For Life clients elect any services or programs external to BCH, the Health For Life Service Coordinator will notify the client in advance of any fees applicable.
