Description of service:
Provision of:
- Outreach nursing to those clients deemed at risk of admission to Austin
Health in relation to existing chronic illness.
This includes:- Monitoring of the individual person's health status
- Providing education in self-management
- Liaising with specialist hospital multidisciplinary teams
and relevant community services to provide early interventions that focus
on preventing or reducing the risk of unplanned hospital presentations.
- A monthly support group for people with chronic heart and lung conditions.
