Banyule Community Health is seeking to employ a full time highly motivated and skilled Occupational Therapist who has experience in the field of community aged care, an ability to work collaboratively with a range of community providers and sensitivity to the needs of the aged, people with disabilities, their carers and people from culturally and linguistically diverse backgrounds.

You will be part of the Community Link Rapid Response Service, a Hospital Admission Risk Program (HARP), which is a partnership between Austin Health, Banyule CHS and other community agencies. This dynamic service is committed to ongoing evolution to meet the chronic disease management needs of clients. You will have the opportunity to be a part of this process, and your creativity, energy and flexibility will be highly valued.

Experience and skills are essential in the areas of home assessment, equipment prescription and follow up. Knowledge of the Aids and Equipment Program and the Department of Veterans Affairs is essential. Familiarity with "The Care Manager" (TCM) software package is desirable.
