Banyule Community Health in cooperation with Austin Health Chronic Disease Program is seeking to employ a highly skilled nurse that is a passionate dedicated self starter to the join the HARP Chronic Disease Management Program in the role of Community Outreach Nurse.

HARP is a partnership program with Austin Health and Banyule Community Health supporting community members with chronic cardiac/pulmonary disease in the community. 

This new three day a week role is complimented by a multidisciplinary team of health professionals who provide support, consultation and care.  The successful applicant will provide health education and support to clients and their carers to improve the self-management of their conditions and enhance individual independence and their capacity to be maintained in the community.
